REQUEST FOR USE OF CHURCH FACILITIES
Northlake Church

Users of church facilities, whether on a donation or free usage basis, agree to the following conditions:
User accepts full responsibility and agrees to hold the church harmless for any injury to persons or property resulting from the negligent acts and/or omissions of officers, agents, and employees during this use.
User agrees to provide adequate adult supervision during entire time facilities are use (no running or jumping on stairs allowed).
User agrees to reimburse church for any damage to church property resulting from the negligent acts and/or omissions of the officers, agents, and employees during this use.
User agrees to make sure all doors and windows are locked and lights are turned off, including restrooms.
User agrees to stay in the area requested and not to use any other facilities.
User must furnish their own equipment needed in connection with the facility’s usage.
User will notify the church of changes or cancellations.
Permission to use facilities is revocable at the discretion of church office. Northlake Church and Three Rivers Christian School activities take priority.
Church needs will supersede prior usage commitments when necessary.
Non-Northlake/TRCS users must provide a certificate of insurance with Northlake Church listed as “additional insured” with a liability limit of $1,000,000.
